Do you want SDS to release your student related disability materials to another agency? Then you will need to complete and sign this form in front of a witness.
Note taker accommodations are approved by Students with Disabilities Services. They are provided as a reasonable academic accommodation for students who have a disability that interferes with their ability to take notes in class.
Priority registration is an accommodation approved by Students with Disabilities Services based on appropriate disability documentation. This accommodation allows the student to sign up for classes on the first day of registration. As with other accommodations, the student is responsible for taking advantage of this priority registration accommodation by clearing any registration 'Holds', prior to the first day of registration.
